Case Manager

Department: ARS - Clinical
Location: Charlottesville, VA

Schedule: Monday-Friday 5a-1:30p, Rotating Saturday 7a-10:30a

ARS Treatment Centers specialize in outpatient treatment for those suffering from opioid use disorder. Our clinic is one of ten locations in the mid-Atlantic region that provides specialized outpatient medication-assisted treatment.

Position Description:

The Case Manager enhances the quality of patient management, maximizes satisfaction and promotes cost effectiveness. The Case Manager will be accountable for the patient care cycle and is expected to assess, plan, implement, monitor and evaluate actions required to meet the client’s health and human services needs.


Duties and Responsibilities:

  • Coordinate and provide care that is safe, timely, effective, efficient, equitable, and patient-centered.
  • Helps patients achieve wellness and autonomy.
  • Facilitate multiple care aspects (case coordination, information sharing, etc.).
  • Helps patients make informed decisions by acting as their advocate regarding clinical status and treatment options.
  • Develop effective working relations and cooperate with medical team, clinical team throughout the entire case management process.
  • Arrange abeyances with social services, health and governmental agencies.
  • Be proficient with State Licensure Standards, CARF Standards, 42 CFR and HIPAA standards and Company policies
  • Adhere to professional standards as outlined by protocols, rules, and regulations.
  • Performs other duties as requested by the Clinical Director.

Qualifications:

  • Bachelor's Degree in related healthcare field or social services
  • Experience in working in case management.
  • Proficient in the use of personal computers
